VOLUNTEER APPLICATION:

A background check is required for all who volunteer with Pop Warner. Anyone who works with the children MUST complete a Volunteer Application every year. Please download this application and give the completed form with a copy of your driver's license to your team head coach if you will be assisting with your child's team.

MEDICAL HISTORY FORM:

All participants must turn in Pop Warner Physical Fitness & Medical History Form prior to being able to participate. Please download this and take it to your doctor to complete. Please make sure both you and child's doctor sign the form.

PLAYER CONTRACT:

Participant Contract and Parental Consent form must be filled out and signed by the parent or legal guardian of each participant in Pop Warner activities.

SCHOLARSHIP POLICY & APPLICATION:

Due to an overwhelming number of scholarships in the 2008 season, SCYF has implemented a scholarship policy and application procedure. All persons wishing to apply for a scholarship for SCYF particpation must abide by the folllowing policy and application procedure. Please download the policy below and fill out the application. The application must be accompanied by supporting information or will not be considered. All applications for scholarships must be competed and approved by the SCYF Board prior to registration or you will not be eligible! Funds are limited, so apply early!
